Clinical Lead (Primary Care)

The Équipe Santé Ontario Cochrane District Ontario Health Team (ÉSOCDOHT) is made up of over 40 health care and community organizations, hospitals, home and community care providers, primary care teams, and lived-experience partners serving both urban and rural communities. We are committed to building a coordinated, patient-centered system that reflects the needs of our diverse population.

We are seeking two part-time Clinical Leads (Primary Care Network) to join our dynamic Ontario Health Team—one Family Physician and one Nurse Practitioner.

The PCN Clinical Lead is a senior leadership role responsible for coordinating and guiding the activities of the ÉSOCDOHT Primary Care Network. This position provides leadership aligned with ÉSOCDOHT’s purpose, values, and objectives while supporting and advocating for family physicians, nurse practitioners, and other clinicians. The Lead will help drive clinical redesign, system improvement initiatives, and integrated care planning.

Responsibilities

  • Identify key priorities, goals, and future direction for the PCN in collaboration with ÉSOCDOHT leadership and the NOSM RISE team
  • Encourage strategic collaboration among interdisciplinary PCN members
  • Attend regular PCN and ad hoc meetings
  • Support co-design and implementation of integrated care models to improve primary care access
  • Drive engagement and participation among primary care providers
  • Serve as liaison between primary care providers and the OHT
  • Represent clinical perspectives during planning and implementation of system-design initiatives
  • Work closely with the Executive Lead, Clinical Project Lead, Project Assistant, PCS, PFAC, and Steering Committee
  • Other duties as assigned

Qualifications

  • MD applicants: Valid CPSO registration in good standing
  • NP applicants: Valid CNO registration in good standing
  • Experience practicing within the Cochrane District is preferred

Other

  • Valid Ontario driver’s license and access to a vehicle
  • Occasional regional travel required

Salary & Benefits
  • $165/hour (10-15 hours per month)
  • Compensation based on ÉSOCDOHT physician leadership rate
  • Time commitment approximately one half-day per week
  • NP remuneration may be paid to the employer for coverage or directly to the NP if performed outside regular hours
