Ways to Participate with the ÉSOCDOHT

There are many ways to participate in shaping the future of health care in the Cochrane District. The Équipe Santé Cochrane District Ontario Health Team offers opportunities for involvement at multiple levels—ranging from community and patient partnership to clinical, organizational, and system leadership. Participation across the OHT is intended to reflect the full diversity of the Cochrane District, including Indigenous and Francophone communities, and other priority populations. Together, these varied perspectives support the development of a more integrated, accessible, and patient-centered health system that improves outcomes and strengthens community well-being. Below you will find descriptions of many of the committees and areas you can participate.

Patient, Family and Caregiver Council (PFACC)

 

The ÉSOCDOHT collaborates with a broad range of community partners to support health and well-being across the Cochrane District. Community partners can be patients, clients, families, caregivers, people with lived experience, and other citizens who interact with health and social services. They are part of our Steering Committee, working group, survey group, or other table where they contribute experience and insight to ensure the voice of the community is brought into the decision-making process.

  • Once a member, you will be able to get involved in the work, share your experiences, gain access to committee and working groups, attend events and provide feedback; insight on all levels of ÉSOCDOHT work that are of interest to you.

Collaboration Council

 

The ÉSOCDOHT works collaboratively with a wide range of health and community organizations to improve care for residents across the Cochrane District. Our partners include hospitals, primary care providers, long-term care homes, community health organizations, mental health and addictions services, public health units, and other local agencies. Together, they help create a connected health system that focuses on patient-centered care, better access to services, and stronger support for individuals and families in the community.

  • Once a member, your organization will be able to get involved in the work, gain access to council meetings, share feedback, support projects and initiatives and stay in the loop regarding progress and updates.

Primary Care Network

 

The Primary Care Network (PCN) provides a unified voice for Physicians, Nurse Practitioners, and Allied Health Professionals within the ÉSOCDOHT. Through direct input to the Steering Committee, Collaboration Council, and working groups, the PCN contributes to decision-making on health care delivery and system planning.

The purpose of the PCN is to strengthen primary care integration across our OHT initiatives by offering expertise, leadership, and active participation in work focused on primary care and related care pathways. The PCN also submits recommendations to the Steering Committee on emerging priorities and community needs.

The ÉSOCDOHT is inviting Nurse Practitioners, Family Physicians, and Allied Health Professionals in primary care to contribute their expertise and help shape the future of health care in the community.
