Community Partnership
Get involved as a member of the community by joining the ÉSOCDOHT Patient, Family and Caregiver Council (PFACC).

- If selected, complete the interview and onboarding process.
- Once a member, you will be able to get involved in the work, share your experiences, gain access to committee and working groups, attend events and provide feedback & insight on all levels of ÉSOCDOHT work that are of interest to you.
Organization Partnership
Get involved as an organization partner, by joining ÉSOCDOHT's Collaboration Council.

- Meet with our Implementation Lead and complete the onboarding process.
- Once a member, your organization will be able to get involved in the work, gain access to council meetings, share feedback, support projects and initiatives and stay in the loop regarding progress and updates.
Clinician Partnership
