Primary Care Network

The Primary Care Network (PCN) provides a unified voice for Physicians, Nurse Practitioners, and Allied Health Professionals within the Ontario Health Team (OHT). Through direct input to the Steering Committee, Collaboration Council, and working groups, the PCN contributes to decision-making on health care delivery and system planning.

The purpose of the PCN is to strengthen primary care integration across OHT initiatives by offering expertise, leadership, and active participation in work focused on primary care and related care pathways. The PCN also submits recommendations to the Steering Committee on emerging priorities and community needs.

Subcommittees & Working Groups

Members of the Primary Care Network may be invited to participate in one or more subcommittees or working groups to support the OHT in fulfilling its role. The mandate and membership of these groups are determined by the PCN.

The PCN meets quarterly to collaborate, share insights, and advance its role in building a stronger, more connected health system.
